Often the last question is sort of a brainstorming task which I don't recall VC discussing. The second and third questions tend to be so mix of business intuition and calculation questions. Since in an interviewer lead style interview, you won't be able to jump back and modify your structure, I suggest spending a lot of time walking through your structure, making it several layers deep, and why you think each piece of requested data would be interesting/necessary.
The first question will just ask you to structure the case.
Real McK interviews tend to have 4 distinct questions and I don't think Cheng follow that well on LOMS. I also really likes the business situation framework compared to others that were offered in case in point.

I found VC most useful for 1) how to talk, 2) how to structure your math. Make sure you pay attention to the last 2 cases in LOMS which are interviewer lead rather than candidate lead case interviews.
